WHAT INSURANCES DO YOU ACCEPT?
We accept all major commercial insurance PPO plans and Medicare Part B.
If you are not sure if your plan is covered – we suggest calling the number located on the back of your insurance card under member services. A representative will go over your specific coverage plan.
DO YOU ACCEPT MEDI-CAL / COVERED CA?
Unfortunately, our private office does not accept Medi-Cal patients.
We are happy to still see patients who have Medi-Cal, so long as they are in agreeance to be a private pay patient.
WHAT ARE THE PRIVATE PAY PRICES?
We offer “Good Faith” prices to our private pay patients, meaning we charge the price that insurances pay us for.
Please call our office for prices. *Prices varies depending on services/testing.
I’M A NEW PATIENT & WAS JUST REFERRED; WHAT NOW?
We can only schedule patients who have a complete referral sent and faxed to our office @ (408) 993-1521.
A complete referral includes:
- Patient Referral form from referring Physican with explanation.
- Patient Medical Records (recent progress notes, imaging, labs, and any hospitalization records)
- Medical Authorization (depending on your insurance)
- Patient Demographics; including insurance information.
WHAT IS YOUR APPOINTMENT /CANCELLATION POLICY?
Patients are given appointment reminders 2 days prior to scheduled appointment via phone, SMS, or email – based on patient preference. If there is no confirmation from the patient, the appointment may be canceled. If a patient fails to confirm/cancel an appointment, an invoice of $50 dollars for No-Show fee will be applied.
Cancellation policy is 24-hours in advance of the patient’s scheduled appointment, otherwise fees may apply.
DO YOU ACCEPT WORKMAN’S COMPENSATION CLAIMS?
Unfortunately, our office does not provide worker’s compensation care claims.
WHAT PAYMENT METHODS DO ACCEPT?
We accept all major credit cards and checks made payable to SKAND Corporation.
